For BCBS National PPO members.
New from BCBS, a website that you can
access your personal health information, view claims for yourself and
minor dependents from the current date to two years prior, (pharmacy
claims are available on the Medco website).
Members may view eligibility for benefits such as medical, dental, vision,
hearing, and pharmacy. Medicare coverage is also displayed on the
eligibility screen. For details and more information,

New Medicare
Prescription Drug coverage will be available beginning
January 1,
2006,
for everyone eligible for Medicare Parts A and/or B. Medicare
prescription drug plans will be provided by private companies and
will benefit primarily those who do not have drug coverage through
an employer.
Based on the
level of benefits that
we currently have you DO NOT want to enroll in this plan. It
would be of no benefit to the UAW/Ford Retirees. As a matter
of fact, if you join a prescription drug plan offered through
Medicare, it may replace your
prescription drug coverage through
Ford Motor Company.

Effective
September 1, 2004, the present BCBS Traditional Plan
will be converted to a PPO Plan. The name of the plan is the
National PPO Plan and will continue to be administered by
BCBS. There will be an in-network benefit level and an
out-of-network benefit level. (For those of you covered by
traditional BCBS you should have received this information in the
mail.) This change will impact only those that are retired and enrolled in the
traditional coverage. Those enrolled in an HMO, Bluelincs or
PacifiCare, will not be affected.
(Active employees enrolled in Unicare are not affected.) |

How Will the
National PPO Plan Work? |

If you
receive covered healthcare services from a participating National PPO plan provider, there is no change to the benefits you have today
under the present Traditional Medical Plan. |
|

If
you receive covered healthcare services from a non-participating
National PPO Plan Provider (an out-of-network provider). you will be
required to pay an additional 10% co-insurance for those covered
services - up to $250 per person or $500 per family each plan year.
In addition, you will be responsible for any charges above the BCBS
maximum allowable payment. |

have Medicare are included in the new PPO and will receive new ID
cards prior to September 1, 2004. |

Covered
services that Medicare pays first are NOT subject to the 10%
co-insurance for out-of-pocket network services and will be
processed the same as under Traditional Medical Plan. |

Mental Health &
Substance Abuse Program |

Non-Medicare
Retirees - Services for psychiatric care and substance abuse
treatment are included in the National PPO Plan. Non-Medicare
retirees must utilize in-network psychiatric and substance abuse
providers to receive full benefits. |

Effective March 1, 2004,
hourly retiree health care changes, such as those listed below, will be handled
by the Retiree Servicing Unit at the National Employee Services Center (NESC) by
calling 1-800-248-4444.

the retiree should contact their health care plan provider (phone number should
be listed on plan member's ID card).
