What is DenteMax?
DenteMax is Blue Cross Blue Shield of Michigan's (BCBSM)
nationwide preferred network of dentists. DenteMax offers you significant
savings as part of the UAW-Ford Enhanced Traditional Dental Plan. Using the
network is easy and entirely voluntary. If you are enrolled in the Enhanced
Traditional BCBSM Dental Plan, the DenteMax option is built-in. There's
nothing else you need to do but select a dentist that is part of the network,
the same way you select any dentist.
No special forms, paperwork or commitments are needed.
Covered family members are free to select the dentist of their choice, in or out
of the DenteMax network. The nationwide network consists of over 52,000
locations! Members can change dentists at any time. |

DenteMax network dentists have agreed to accept 20-32%
less " payment than the average non-network dentist. This means you save on
any of your out-of-pocket costs. |

The DenteMax predetermination feature protects you
from balance billing surprises. If a network dentist does not obtain benefit
predetermination on amounts over $200, you will not be responsible for
unapproved charges for covered services. This feature does not apply, to
non-network dentists and is not a guarantee of payment. |

DenteMax network dentists go through one of the most
rigorous credentialing processes in the industry, not just initially, but
every three years;, These processes meet 100% of the quality; standards of the
prestigious National Center for Quality Accreditation (NCQA). This offers
you and your family members added peace of mind in addition to significant
savings when going to a DenteMax network dentist.
There are over than 52,000 dentist
locations nationwide. Ask your dentist if he or she is in the network, if not,
you can nominate your dentist by calling DenteMax at 1-800-752-1547.
If Your Dentist
Requires More Information
Your
dentist may obtain an instruction sheet to request predetermination of services
that cost more than $200 and to clarify billing procedures by calling
1-800-482-5141 or 1-800-482-4047.

How Your Dentists Get Paid
DenteMax and other
participating dental ! providers will submit a claim for your dental services
directly to Blue Cross Blue Shield of Michigan, who makes the payment directly
to the dentist. The dentist will bill you for your co-payment. Those dentists
who are not DenteMax dentists and who do not participate with BCBSM may bill you
for amounts in excess of approved amounts.

policies and negotiated Agreements by which these benefits are provided.
If there is any difference between the Plan texts and this information,
the Plan text and negotiated Agreements always will govern. Ford Motor
Company reserves the right to end, suspend and amend plans, subject to
the applicable Collective Bargaining Agreement. Amendments also will be
made to comply with applicable statutes and regulations. If changes are
